New Satisfactory Academic Progress (SAP) Appeal Online Form - Advising Network Meeting Follow Up [email 3/9/23]

The Office of Financial Aid & Scholarships has created a new online SAP appeal form in place of the old PDF. Here is a link to the form: SAP Appeal Form. Students must log in with their CatID to access the form. Everything required for the appeal has now been embedded within this online form, but students can still submit documents via email, mail, fax, or dropping them off at our front desk. We will also still accept the old PDF if students are unable to complete the online form.

Some important notes for advisors:

• The new form has the option to upload a copy of the Plan of Study. If students select this option, the plan must be physically signed by the advisor; we can't accept electronic signatures.
• If students don't want to upload the plan to the online form, they can also email the plan to jennifer.sullivan@uni.edu. It must be signed by their advisor (no electronic signatures). If the advisor is unable to sign the plan, they can email it to jennifer.sullivan@uni.edu and confirm they approved the plan. 
• The student can also submit a signed plan via mail, fax, or by dropping it off at the front desk.
